Transfering of US stock to IG?

Hi, I have a form to fill out from my US brokerage account with Fidelity to transfer my US shares to IG,
Firstly, can IG accept a transfer of US stock?
Secondly, the form requires teh following, there is also a screen grab attached - any help will be appreciated!
