Transferring share in from Corporate incentive plan

HI I opened an account to transfer shares in from a global long term incentive plan from my old company. I can't find any info or get through via email or phone - I need some key information to provide eg brokerage email, Swift ID and Crest ID - any ideas where I can get this data? Thanks
