Limit Order removed without being closed

A Limit Order position that has been open for weeks is no longer displayed (without being filled).  I had set up the order in May and has been sat in the Orders tab up until this week when it is no longer set.  There was no expiry date set on this order, and no evidence trail in the history - any advice on how and why this would be removed without my knowledge?
